Position Overview
A healthcare plan is seeking an Account Manager to manage and maintain existing referral relationships within the Managed Long-Term Care (MLTC) and MAP space. This role focuses on relationship management not new business development working closely with home care agencies and community partners to maintain strong referral pipelines.
This is a highly field-based role requiring strong people skills consistent follow-up and relationship-building within the community.
Key Responsibilities
Maintain and strengthen existing referral relationships (primarily home care agencies)
Manage a strong existing book of accounts and ensure ongoing engagement
Conduct regular visits to home care agencies and community partners
Educate referral sources on plan benefits and enrollment process
Ensure qualified patients enroll in appropriate healthcare plans
Track outreach activities and maintain thorough follow-ups
Support membership growth through relationship management
Coordinate with internal teams on referrals and member needs
