Position Summary
ELFHCC is seeking a Registered Dietitian (RD/RDN) to join our interdisciplinary care team. The Dietitian will provide nutrition assessment counseling and education to patients across the lifespan supporting chronic disease management preventive care and overall wellness. This role works closely with medical providers nursing staff and care teams to deliver integrated evidence-based nutritional care.
Key Responsibilities
- Conduct comprehensive nutrition assessments and develop individualized care plans
- Provide nutrition counseling for chronic conditions such as diabetes hypertension obesity and cardiovascular disease
- Support maternal pediatric and womens health populations as applicable
- Serve as the clinical lead and program manager for the Medicare Senior Weight Loss Program including coordination tracking outcomes and supporting program growth
- Collaborate with providers and clinical staff as part of an integrated care team
- Document care in the electronic health record in a timely and compliant manner
- Participate in patient education group visits and community outreach as needed
- Maintain compliance with all regulatory quality and documentation standards
Qualifications
- Registered Dietitian (RD/RDN) credential required
- Licensed or eligible for licensure in Pennsylvania
- Experience in primary care community health or outpatient settings preferred
- Strong communication and patient education skills
- Ability to work collaboratively in a fast-paced team-based environment
- Bilingual skills a plus (not required)
