Job Summary

This position is PRN for Polk Hardee Highlands county.
What youll do:
- Under the direction of the Access Manager and the Hospital Access Navigator the LPN is responsible for identifying Patient/Family needs and coordinating Interdisciplinary Team (IDT) provision of services under physicians orders.
- Utilizes monitoring intervention and teaching skills to maximize patient comfort and enhance the quality of life for the Patient/Family unit.
- The LPN is supervised and collaborates with Primary RN and/or CTM.
What youll Need:
- Education and/or Experience: Associates degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
- Continuing Education: As required for licensure.
- Licensure:
- Valid Florida LPN License
- Valid Florida State Drivers License
- Certifications:
- Valid CPR certification from the American Red Cross or American Heart Association
- Current IV Therapy certification preferred. Must obtain the certification within six months of hire date.
