Position: Assertive Community Treatment (ACT) Case Manager

Location: Hays Kansas HPMHC

Pay: $25 per hour Incentive Pay $500 SIGN-ON BONUS!

Job Type: Full-time In-person

Schedule: typical hours Monday-Friday 8am-5pm On-call rotation

What Youll Do:

  • Provide community-based case management and rehabilitative services to adults with serious mental illness and co-occurring conditions.
  • Deliver most services in clients homes and throughout the community to promote independence and recovery.
  • Develop implement and maintain individualized service plans in collaboration with consumers families and treatment team members.
  • Coordinate care with hospitals medical providers community partners and other stakeholders to ensure continuity of care
  • Support clients in improving life domains including housing employment education social relationships health and legal matters.
  • Practice assertive outreach to reduce hospitalizations increase housing stability and improve overall quality of life.
  • Provide mobile crisis response and participate in an after-hours on-call rotation.
  • Transport clients as needed and respond to community-based emergencies.
  • Complete timely documentation and actively participate in supervision and team meetings.

Requirements
  • Experience counts! We accept equivalent work experience in human services (teaching daycare CNA etc.) or a BA/BS degree. One year of related work one year of college.
  • Demonstrated ability to work effectively with individuals experiencing serious mental illness functional impairments and complex needs. Strong interpersonal communication and crisis intervention skills.
  • Must pass KBI criminal history background check DCF Child/Adult Abuse and Neglect Registry checks professional background checks and complete required state-approved training.
  • Valid drivers license with a driving record consistent with insurability (no substance-related incidents within the past five years).
  • Ability to travel regularly (Center vehicle provided) and provide client transportation.
  • Proficiency with computers and general office technology.
