Job Summary

JOB SUMMARY
Under the administrative supervision of the Executive Director or designee provides general medical care and treatment to recipients in collaboration with the physician. Majority duties are often performed independently.

ESSENTIAL JOB FUNCTIONS: List all essential job duties. 1. Responsible for the primary health care of assigned consumers.
2. Performs history and physical examinations and preventive health measures within prescribed guidelines in collaboration with a physician.
3. Orders interprets and evaluates diagnostic tests to identify and assess recipients clinical problems and health care needs. Orders laboratory tests x-rays and other diagnostic procedures.
4. Observes recipient progress records physical finding and formulates plan and prognosis based on recipients condition. Discusses case with physician and other health professionals to prepare comprehensive recipient care plan.
5. Submits health care plan and goals of individual recipients for periodic review and evaluation with the physician. May recommend changes to the plan of care. Prescribe or recommend medication or other forms of treatment such as physical therapy diet inhalation therapy or related therapeutic procedures. Prescribes or recommends continuation or discontinuation of medications and treatment.
6. May refer recipients to physician for consultation or specialized health resources for treatment.
7. May assist the psychologist in performing mental status assessment and behavioral evaluations. Performs crisis intervention using verbal and nonverbal techniques.
8. May have on-call responsibilities.
9. Immediately reports serious incidents serious incident allegations incidents or sensitive situations to supervisors.
10. Assists in the development of nursing policies and procedures and in the orientation of new nurses.
11. Compliance with attendance rules and the ability to work on a regular basis.
12. Attends regularly scheduled clinical meetings and supervision attends professional development activities and training.

13. Performs other job-related duties as assigned.

ADDITIONAL TASKS:

1. May conduct workshops for recipients and staff on health-related issues to include health maintenance medications and medication administration.
2. May precept nursing students and nurse practitioner students.
3. May be assigned as a chair or part of an agency committee(s).
4. May serve as the agency Bloodborne Pathogen Coordinator and teach the Bloodborne Pathogen Standard workshop and makes recommendation concerning the agency infection control posture. May conduct and interpret Mantoux Tests (TB).

Knowledge of both verbal and written English at the Master degree level.
Ability to react appropriately in emergency situations.
Ability to perform and interact as part of a multi-disciplinary team in order to provide primary care to recipients.
Requires knowledge of serving residents with developmental disabilities.
Ability to perform and interact as part of a multi-disciplinary team to provide care to clients.
Ability to secure the cooperation of and work effectively with others.
Ability to prepare accurate and timely reports.
Ability to effectively communicate both orally and in writing.
Knowledge of teaching and consulting techniques.
Knowledge of the Agencys policies and procedures.

QUALIFICATIONS AND EXPERIENCE

Masters degree in Nursing from an accredited nurse practitioner program and currently licensed in New York State as a family practice nurse practitioner. Current DEA certification. ANCC certification is an asset.
Two years experience providing nursing services preferred. Must be eligible for full and unconditional participation in the Medicaid and Medicare programs. May be required to possess a valid NYS driving license and a clear driving record.

Continued employment is based on the ability to maintain professional licensure registration accreditation and participation in the Medicaid and Medicare programs.
