SCHEDULE: Part time 20 hours per week Day/Evening Shifts
SUMMARY
As a Certified Home Health Aide (HHA) you will provide care to clients in their home following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.
RESPONSIBILITIES
Patient Care & Service. Promote and restore patients health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care light housekeeping meal preparation and errands
Communication. Maintain effective communication to convey patient health status treatment plans and progress; communicate with office and supervisory staff regularly
Documentation. Accurately and completely document client care on activity sheet
Compliance. Adhere to required department and system protocols HIPPA regulations local state federal requirements
Required Qualifications
Compassionate patient focused and dependable professional
Critical thinking skills decisive judgment and the ability to work with minimal supervision
Exceptional documentation and communication skills
Ability to read write and speak English ability to follow verbal and written instructions
Valid NYS Drivers License
HHA Certified or Ability to meet one of the following eligibility requirements for Competency:
Currently certified CNA along with completion of written test and one day HHA Skills competency class within 4 days of hire.
Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program
The Certified HHA who wishes to be re-employed after a two-year lapse in employment as a HHA must take the 1 Day Skills Competency Class. They must show completion of written test prior to taking the class and pass the one day HHA Skills competency class within 4 days of hire and before working as an HHA. If the candidate has worked as a Certified HHA within the past 24 months and is actively certified no class is needed.
Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 4 days of hire.
Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 4 days of hire.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
