About the Role:
The Occupational Therapist in the Health Connect Program provides preventive care chronic health monitoring and functional support to clients living in supportive housing. Using a traumaâinformed and personâcentered approach the OT conducts health assessments develops individualized care plans teaches independent living skills and facilitates community integration. The role focuses on enhancing clients ability to live independently while managing mental health needs substance use recovery and preventive healthcare. The OT collaborates closely with a multidisciplinary team to ensure holistic coordinated care.
Primary Responsibilities:
Conduct comprehensive psychiatric medical and social health assessments for new and existing clients.
Monitor chronic health conditions and ensure appropriate interventions and followâup care.
Develop and implement individualized care plans that promote independence and community integration.
Collaborate with LCSWs RNs social workers and peer specialists to deliver coordinated holistic services.
Provide health education and training to clients and staff emphasizing prevention and chronic disease management.
Participate in the Clinical HighâRisk Framework and weekly meetings to assess needs prioritize care and monitor progress.
Engage clients to support program compliance and participation in healthâpromoting activities.
Liaise with external healthcare providers and community resources to enhance care outcomes.
Document assessments interventions and progress in accordance with program standards.
Teach and reinforce IADLs such as home management budgeting meal preparation and grocery shopping.
Assist clients in developing routines that promote stability and independence.
Support clients in accessing community resources including transportation healthcare and social services.
Promote social engagement and community participation to strengthen support networks.
Ensure home safety through costâeffective environmental modifications.
Provide client advocacy education and support.
Perform other related duties as assigned.
