Position Summary

The Nurse Intake Coordinator (LPN) serves as the first clinical point of contact for patients referred to home health or hospice services. This role is responsible for coordinating admissions reviewing referrals for appropriateness ensuring regulatory compliance and facilitating a smooth transition of care. The Intake Coordinator works closely with referral sources patients families physicians and the interdisciplinary team to ensure timely and accurate onboarding of new patients.

Key Responsibilities

Referral & Intake Management

  • Receive review and process referrals for home care and hospice services.
  • Verify eligibility insurance coverage and benefits.
  • Ensure required documentation (physician orders face-to-face documentation clinical records) is complete and compliant.
  • Prioritize referrals based on urgency and clinical need.
  • Coordinate start-of-care scheduling with clinical staff.

Clinical Review

  • Perform initial clinical assessment of referral information to determine appropriateness for services.
  • Identify patient needs level of care and special equipment requirements.
  • Communicate with physicians to obtain clarifications or additional orders as needed.
  • Ensure hospice eligibility criteria are met when applicable.

Coordination & Communication

  • Serve as liaison between referral sources patients/families and the care team.
  • Provide clear explanation of services coverage and expectations to patients and families.
  • Communicate admission details to field clinicians and interdisciplinary team members.
  • Maintain ongoing communication to ensure seamless transition into services.

Documentation & Compliance

  • Maintain accurate intake records in the electronic health record (EHR).
  • Ensure compliance with Medicare Medicaid and other payer guidelines.
  • Adhere to state and federal regulations governing home health and hospice care.
  • Track referral trends and maintain intake logs as required.


Qualifications

Qualifications

Required:

  • Active unencumbered Licensed Practical Nurse (LPN) license in the state of practice.
  • Minimum 12 years of clinical nursing experience.
  • Experience in home health hospice case management or intake coordination preferred.
  • Knowledge of Medicare/Medicaid regulations for home care and hospice.
  • Strong assessment organizational and communication skills.
  • Proficiency with electronic medical records (EMR/EHR) systems.

Preferred:

  • Experience with insurance verification and prior authorizations.
  • Knowledge of hospice eligibility guidelines.
  • Bilingual skills (if applicable to the service area).

  • Standard business hours with possible on-call rotation depending on agency needs.
