Job Summary

Compensation Range: $23/hr - $26/hr commensurate with experience

Overview:

In collaboration with the Mayors Office of Criminal Justice (MOCJ) the Housing Works Justice Initiatives is launching a new pilot program CREATE: Cannabis Reentry Education And Training Experience. This program is designed to provide individuals 21 years of age and older with previous justice-involvement with specialized classroom and on-the-job training to prepare individuals for a career in the newly emerging Cannabis Retail industry. It is an eight-week paid training program that includes two-weeks of classroom training and six-weeks of on-the-job training in a designated NYC licensed cannabis dispensary. Reporting to the Program Coordinator the Case Manager will be primarily responsible for engaging in outreach and recruitment activities conducting program enrollment screenings and comprehensive intakes develop and review service plans assist with benefits and entitlements enrollment and connecting individuals to internal and external wraparound support and healthcare services. Candidates for the Case Manager position will ideally have experience working with individuals or communities impacted by the criminal legal system knowledge of the retail or cannabis industry skilled in providing care coordination services and a passion for participating in the development of this newly emerging sector.

Responsibilities:

Primary:

  • Provide case management services to actively enrolled clients.
  • Participate in outreach and recruitment activities.
  • Conduct initial screenings with clients referred to gauge interest and eligibility for the program.
  • Conduct comprehensive assessments with newly enrolled clients.
  • Assist clients with benefits and entitlements enrollment and obtaining vital documents.
  • Connect clients to wraparound support and healthcare services needed such as primary care mental health services substance use treatment etc.
  • Co-develop treatment plans and review with clients every 60 days.
  • Facilitate work readiness or soft skills workshops.
  • Conduct regular consistent case management sessions with clients assigned to caseload.
  • Participate in case conferences or care coordination activities.
  • Engage in crisis intervention activities as needed.
  • Utilize database to complete progress notes and other client-related data.
  • Actively participate in individual supervisions with assigned supervisor.
  • Promote Housing Works core values in day-to-day activities.
  • Performs other tasks as assigned by supervisor.

Secondary:

  • Provide culturally competent services in accordance to Housing Works policies and practices.
  • Ability to work with clients with diverse clients with varying levels of service needs.
  • Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.

Requirements:

  • High School Diploma or equivalency required.
  • Bachelors Degree in a related field preferred.

The following are qualifications and requirements of the position:

  • Minimum 1 year experience providing case management or direct care services.
  • Experience working with individuals or communities impacted by the criminal legal system.
  • Proficient knowledge of NYC support services and city state and federal benefits.
  • Strong written documentation and communication skills.
  • Good problem-solving skills and ability to work autonomously.
  • Microsoft Office Suite particularly Excel required.
  • Experience utilizing electronic medical record (EMR) systems.
  • Preferably bi-lingual in Spanish.
  • Dependability and reliability to arrive and depart timely for scheduled shift.
  • Ability to frequently travel within the boroughs via public transportation.
