About Fort Logan:
The Colorado Mental Health Hospital at Fort Logan (CMHHIFL) is one of two state hospitals serving our community. The vision of CMHHIFL is to provide the highest quality mental health services to persons with complex serious and persistent mental illness within the resources available. Our mission is to support those individuals in achieving their recovery goals and reaching their full potential. CMHHIFL provides inpatient treatment to adult patients generally between the ages of 18-59. The hospital currently has 138 adult inpatient beds over six treatment milieus serving civil and forensic individuals. Medical services occupational therapy neuropsychological-rehabilitation services and trauma-informed care services are available for all patients throughout the hospital.

Description of Job
On Site: Monday - Friday 8:00 am to 4:00 pm

Brief Duty Description:
This position is entrusted with the creation of and subsequent updating of a legal database for all patients that houses pertinent admissions and court related information. Ensures patient legal status throughout the duration of admission to CMHHIFL is up to date and accurate and monitors all related legal documentation as necessary.
Coordinates with court services forensic services and courts regarding the scheduling of legal proceedings and provides this information to Treatment Team members as necessary. Manage and consolidate all official responses to the County Attorneys Office regarding legal questions about CMHHIFL patients.
Create and maintain a database with the legal status of every patient ensuring all information is up to date accurate and disseminated to relevant staff throughout the hospital. Implement a mandatory daily auditingprocedure for all received court orders the daily discharge census and reports mailed to the courts to verify compliance and accuracy.
Prioritize the drafting and mailing of all necessary legal letters to the criminal courts defense attorney and district attorney promptly upon a patients placement acceptance requesting case dismissal.
Allocate dedicated resources to proactively find and resolve outstanding patient warrants including coordinating court appearances to actively seek case dismissal or resolution for increased efficiency.
Formalize the liaison role to facilitate clear and timely communication between the County Attorneys office psychiatrists and patient legal representatives. Standardize the communication process with County Sheriffs Departments regarding the coordinated delivery of patient property to CMHHIFL upon admission or transfer.
Adopt a system-level solution to manage and automatically flag instances where multiple court orders are received for a single patient in a day thereby mitigating potential errors in the electronic medical record and legal status entry screens.
Ensures all updated court orders writs evaluations and other legal information are scanned into the electronic medical record and subsequently routed to the appropriate unit-level staff.
This position is responsible for reaching out to external stakeholders in order to obtain as much information as possible regarding patients referred to or ordered to CMHHIFL. This includes jails hospitals courts physicians families etc.
Once obtained this position works independently using professional clinical skills knowledge and experience to review and summarize the information into a pre-assessment. Collateral information includes current presentation historical information issues of concern medical conditions and special accommodations. Once all information is received this position is responsible for reviewing medical information with the medical physicians. If and when the patient is cleared disseminate the information to the treatment team. This position also arranges doc to doc and nurse to nurse consultations.
There are other Health Professionals in the Admissions Department and referrals often carry over from previous days or previous shifts. This position must stay abreast of other cases be able to quickly pick up cases previously started by other Health Professionals and be able to accurately disseminate the addition provide accurate and concise pass on notes for oncoming shift these cases this position is responsible for having a comprehensive overview of the patient in order to relay important information to the team staff and other departments.
Once the patient is admitted this position is responsible for updating the Electronic Health Record meeting with the patient and reviewing and signing all legal admission paperwork. This position also updates hospital and team census and is the liaison between CMHHIFL CMHCs and RAEs notifying them of their patients admission status.
Negotiate with County Sheriff Departments to secure the timely pick-up of patients from CMHHIFL. Coordinate with the Admissions Director to arrange safe and secure transport of patients to Colorado Jails and RISE Programs.
Apply proficiency in Admissions policies to locate and provide accurate information and reference materials to all external stakeholders seeking guidance. Collaborate with medical psychiatry and nursing staff to ensure all preparatory steps are completed before executing admissions discharges and transfers. Utilize knowledge of Colorado Mental Health Services/Behavioral Health Administration to assess service needs and direct individuals seeking services to the appropriate level of care

HEALTH PROFESSIONAL II (C7C2TX)Experience Only:
Five (5) years of relevant experience with Hospital Admissions Legal Services or other closely related field to include experience working with courts knowledge of the legal system and expert level data collection .
OR
Education and Experience:A combination of related education in Criminal Justice Psychology Social Services and/or relevant experience in an occupation related to the work assigned equal to five (5) years.
Preferred Qualifications:
Data Collection reporting and knowledge of the legal system
Full background check and reference check required
