Case Manger Loveland Outreach
- Salary Range:$24.04 - $28.85 per hour based on relevant years of experience
- Location:Loveland Colorado
- Status:Full time 40 hours per week
- Hours: Monday - Friday
Role Overview:
Responsible for coordinating integrated (i.e. substance abuse and mental health) case management services for our unhoused and unsheltered populations. Case managers engage assess plan advocate coordinate and link clients to formal/informal activities services and/or support within the community that promote wellness and safety. Case managers provide crisis therapeutic interventions.
Essential Duties:
- Coordinates a wide variety of services for clients to receive benefits from internal and external service providers regarding government-based benefits.
- Provides assistance and coaching to clients with daily life activities such as bill paying/managing finances expense tracking completing applications for services and other needs.
- Coaches clients on behaviors to maintain daily life activities.
- Interacts regularly with federal state and local entities to create opportunities to deliver services to clients. Represents clients with these entities to obtain services amend services or re-instate services.
- Perform case management services as needed to assist clients to meet their goals as stated in their service/treatment plans. Refer clients to supportive services such as vocational medical social security housing authority etc.
- Assist clients in improving their well-being and quality of life by coaching in behavioral skills; educate clients to make informed choices; develop clients coping skills; help clients learn to problem solve and negotiate tasks of daily living.
- Documents services in Health Record by completing progress notes case management notes letters faxes and phone calls.
- Maintain close and cooperative relationships with community agencies and other public agencies and private business such as landlords and banks in carrying out supportive services.
- Advocate for clients in many different forums including the federal state and local court systems with family school system etc.
Required:
- Bachelors degree in behavioral science required
- At least one year of experience in directly relevant experience in the Mental Health and/or Substance Use Disorder Field required
Licenses and Certifications:
- Valid Colorado drivers license or ability to obtain within 30 days of hire and a clean driving record
- CAS and/or ability to obtain CAS credential preferred
Highly Desired:
- Prior experience working directly with individuals experiencing homelessness including hands-on case management coordinating services securing housing and connecting individuals to behavioral health and community resources is highly desired.
- Bilingual/bi-cultural preferred.
