The Role: Clinical Anchor & Advocate As a Per Diem Private Duty RN you are the heart of our clinical operations. You will manage complex cases and provide high-level care that keeps our clients safe and healthy at home.

Primary Accountabilities:

  • Clinical Oversight: Initiate and manage new cases conduct home evaluations and provide clinical supervision for our team of CNAs.
  • Complex Nursing Care: Administer physician-prescribed medications perform advanced wound care and manage medical devices.
  • Patient Advocacy: Accompany and advocate for clients at physician appointments and lead hospital discharge planning for seamless transitions.
  • Education & Empowerment: Provide essential teaching to patients and families to support informed decision-making and self-care.
  • Documentation: Utilize EMR software to communicate clinical updates to the DON ADON and families in real-time.

Who You Are We are looking for a Servant Leader with deep clinical roots and a commitment to professionalism.

Required Qualifications:

  • Nursing Background: 5 years of RN experience in Rehab Long-Term Care or Assisted Living.
  • Case Management: 5 years of experience in client care planning and management.
  • Credentials: Active Rhode Island RN License CPR Certification valid drivers license and an insured vehicle.
  • Critical Thinking: The ability to rapidly assess symptoms and make high-stakes decisions in a home environment.

Physical & Travel Requirements:

  • Ability to lift 50 lbs assist with transfers and travel across Providence Kent and Washington Counties.

Believe it or not our story begins when our CEO Samanta (Sam) a hospice registered nurse was challenged by her husband Caleb to Do something about it!

Working as a hospice nurse caring for the elderly with terminal illnesses Sams greatest frustration was that the caregivers hired by her patients families had no idea what they were doing. It negatively affected her patients well-being and comfort. After investigating the matter she found that Connecticut set no standards for home care agency owners or the caregivers they hired to care for others. As a result anyone could be employed as a caregiver with little to no experience training certification or supervision and this was often the case and still is today.

After weeks of complaining about this to her husband being the no-nonsense type of guy he is he challenged her to do something about it. Little did he know that she in turn would convince him to leave his engineering career and help her start a home care agency providing the elderly with high-quality caregivers who would keep them well cared for and safe at home.

Since then Sam has continued to share her vision with countless other professionals. Convincing them to leave their careers and join her fight to be the Gold Standard for home care helping the elderly age in their homes comfort as they so often prefer.
