The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are essential. This position requires experience in peritoneal dialysis in an outpatient setting and the ability to educate and train patients in peritoneal dialysis.

Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Minimum 3 years of full-time clinical nursing experience.
- Minimum 2 years of current experience in peritoneal dialysis in an outpatient setting.
- Prior experience in education and training patients in peritoneal dialysis within the last 3 years.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
- Active Registered Nurse license in the state of working.

- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
Special Considerations:
- On-call required: 12-hour on-call shift every other Sunday and a 3-hour on-call shift following regular shift approximately 2 times weekly.
- Mandatory float facility.
- RTO: Maximum of 5 days with no more than 2 consecutive days.
- No RTO the day prior to or after a holiday.
- No RTO on weekends.
- Scrubs: Any color not provided.
