Location:In-person opportunity in Oak Ridge TN

Hours: Full-time (Mon - Fri) Exempt

Rate of Pay: $23.50 hourly / $48900 annual (BOE)

Emory Valley Center is seeking a dedicated and experienced professional to join our team as the Case Management Director. This leadership role offers an exciting opportunity to oversee the coordination of services that directly impact the lives of individuals supported by our organization. KEY RESPONSIBILITIES*:

  • Lead and supervise the Case Management team including the Assistant Director and Case Managers ensuring effective coordination and delivery of case management services.
  • Oversee the assessment and treatment planning processes to develop personalized support plans for individuals served.
  • Coordinate with internal staff and external professionals to facilitate comprehensive person-centered services.
  • Maintain and ensure the accuracy confidentiality and completeness of personal records and files related to client services.
  • Monitor and evaluate the effectiveness of case management strategies making improvements as needed.
  • Ensure compliance with organizational policies state and federal regulations and best practices in case management.
  • Provide training mentorship and performance evaluations for team members to foster professional growth.
  • Collaborate with other departments to support holistic service delivery and organizational goals.
  • Advocate for individuals needs and promote a person-centered approach within the organization.

Understanding and commitment to person-centered support for people with intellectual and developmental disabilities.
  • Knowledge and understanding of company operations and DDA CHOICES and Employment and Community First CHOICES.
  • Ability to present facts and recommendations effectively in oral electronic and written form.
  • Ability to exercise good judgment responsibility and independent action.
  • Excellent crisis management interpersonal communication time management and organizational skills.
  • Uphold the strictest of confidentiality and follow the guidelines related to the Health Insurance Portability and Accountability Act (HIPAA)/HITECH and Family Educational Rights and Privacy Act (FERPA).
  • Utilize Electronic Health Record (EHR) as required for Company operations.
  • Advanced computer skills.
  • Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty).
  • Understanding of Person-Centered Practices/Thinking.
Requirements

MINIMUM REQUIREMENTS:

  • Bachelors degree plus one year of relevant experience; or Associates degree with two (2) years of relevant experience; or Licensed Practical Nurse (LPN). A Medical Assistant certificates may be accepted as an alternate to LPN with at least two (2) years of experience.
  • A minimum of one-year experience in the field of DDA.
  • Case Management experience preferred.
  • Management experience preferred.
  • DDA and Managed Care Organization (MCO) designated online training First Aid CPR and Medication Certification course within first 90 days of employment and any other required training.
  • Person Centered Thinking Training.
  • Emory Valley Center Orientation.
  • Skills System training.
  • Valid TN drivers license and a good driving record.
