As a RN Care Manager at Mercy youll be a vital part of an interdisciplinary team dedicated to guiding patients safely and compassionately through their hospital journey. From admission to discharge youll assess needs coordinate services advocate for patients and families and ensure every individual receives a smooth wellplanned transition to the next level of care.In this role youll use your clinical expertise and critical thinking skills to support patient safety experience and quality outcomes. Youll help manage lengthofstay goals implement best practices and contribute to a care management model that consistently delivers excellence.

Position Details:
Required Education:
Graduate of an accredited School of Nursing required
Bachelors Degree of Science in Nursing minimum required
Experience:
2-3 years acute care hospital setting preferred.
Care Management or Utilization Management experience preferred
Licensure:
Current License in the state of employment required
Certifications:
BLS (CPR) at hire date required or within 90 days of hire
Certification in Case Management Preferred
