Summary
Under the direction of the Administrative Director of Cardiovascular Services the Heart Wellness Registered Nurse (HWRN) is accountable for following patients enrolled in the Heart Wellness Program via Tele-management and screening and following hospital patients for the core measures of heart failure. The HWRN functions as a clinician and educator in the provision of patient care for the adult patient population ages 18 and up and oversight of program outcomes.
Education
Graduate of an accredited Nursing program. Bachelors in Nursing (BSN) preferred.
Certification Registration & Licensure
Certification required to obtain: Certified in Basic Life Support within 3 months of hire date.
Licensure required: Licensed as a Registered Nurse in the State of NH.

A minimum of 3 years acute care clinical experience in cardiovascular nursing.

Responsibilities
Manages care of the complex cardiac patient within the scope of the Heart Wellness Program.
Utilizes tele-management to identify problems and avoid hospital admissions and intervenes as
appropriate according to Heart Wellness Program protocols.
Manages medication for the inpatient and outpatient population making necessary adjustment or recommendations in conjunction with the Primary Care Provider or Cardiologist.
Serves as a clinical resource for organization in area of expertise.
Identifies potential patients to be managed through the Heart Wellness Program by a review of all new
admissions.
Provides educational sessions using adult learning principles to support patients families and staff and
including outside organizations continuing the care of patients.
Collaborates with other disciplines to achieve patient care and organizational goals.
Produces monthly newsletter and conducts monthly educational and support programs.
Takes ownership and responsibility for individual performance goals.
Involves self in departmental team and committee goals.
Completes departmental and hospital requirements.
Performs other duties as assigned.
