Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) for a 2nd shift contract opportunity at Chapel Manor. The ideal candidate will provide quality patient/resident care administer medications and support the healthcare team in delivering safe and effective nursing services.

Responsibilities

Provide direct patient/resident care in accordance with facility policies and care plans
Administer medications and treatments as prescribed
Monitor and document patient conditions vital signs and progress
Collaborate with physicians and interdisciplinary teams
Assist with admissions discharges and transfers
Respond promptly to resident needs and changes in condition
Maintain accurate and timely documentation
Ensure compliance with infection control and safety guidelines
Supervise Certified Nursing Assistants (CNAs) if applicable

Requirements

Active LPN or RN license (State-required)
Ability to work 2nd shift (3:00 PM 11:00 PM)
Strong clinical and assessment skills
Ability to stand walk lift and assist residents as needed
Professional communication and teamwork skills

Preferred

Long-term care / skilled nursing experience
CPR/BLS Certification
Experience with EMR documentation
