DescriptionJob Title: Case Manager Teen Pregnancy Program
Employee Testimonial: Project ELECT (Education Leading to Employment and Career Training) Case Manager provides comprehensive case management and support services to pregnant and parenting teens to ensure their educational and personal success. This role focuses on assisting students in overcoming barriers to graduation and achieving self-sufficiency through education advocacy and access to resources.
Duties and Responsibilities:
Case Management Services:
- Provide individualized case management to pregnant and parenting teens enrolled in Project ELECT programs.
- Conduct needs assessments and develop goal-oriented service plans in collaboration with program participants.
- Monitor progress and provide ongoing support to ensure successful completion of educational and personal goals.
Education and Support:
- Assist participants in accessing and navigating educational programs childcare services healthcare and other resources.
- Facilitate workshops and group sessions on topics such as parenting skills financial literacy career readiness and health education.
Resource Coordination and Advocacy:
- Coordinate referrals to community-based services including housing counseling healthcare and employment programs.
- Advocate for students within school systems and community agencies to address barriers to education and well-being.
Collaboration and Partnership:
- Work collaboratively with school personnel community organizations and other stakeholders to support program goals.
- Maintain strong relationships with partnering agencies to expand resource options for participants.
Program Compliance and Documentation:
- Maintain accurate confidential case records and documentation in compliance with program requirements and regulations.
- Prepare and submit timely reports on program activities participant progress and outcomes.
Program Development and Outreach:
- Assist in planning and implementing program initiatives special events and outreach efforts.
- Promote Project ELECT services to schools community organizations and prospective participants.
Qualifications:
Education:
- Bachelors Degree or associates degree in social work counseling education or related field required.
Experience:
- Minimum of 2 years of experience in case management youth services or working with pregnant and parenting teens. Experience in educational settings is a plus.
Skills:
- Strong interpersonal organizational and communication skills.
- Ability to build rapport with diverse populations and maintain a non-judgmental supportive approach.
- Knowledge of community resources and services for youth and families.
- Proficiency in documentation reporting and using case management systems.
Additional Requirements:
- Current Act 114 Act 34 Act 151 and NSOR Clearances required prior to an offer (Must be dated within 1 year of hire date).
- School Personnel Health Record (Form H511.340) must be completed and received by Human Resources prior to hire date.
- Valid drivers license and reliable transportation.
Working Conditions: This position operates in a combination of school office and community settings with frequent travel to program sites and participant homes. Occasional evening or weekend hours may be required. This position may involve standing walking and active engagement throughout the day.
