Job Summary
A reputable long-term care facility in Rutland Vermont is seeking an experienced Travel Licensed Practical Nurse (LPN) for a night-shift assignment. This role involves providing direct nursing care supervising nursing assistants and ensuring high-quality resident care in accordance with regulatory and facility standards.
Responsibilities
Supervise and direct daily activities of nursing assistants in compliance with facility policies and regulations
Coordinate shift activities programs and services with nursing and support staff
Complete and maintain accurate resident documentation for admissions transfers and discharges
Communicate with physicians regarding treatment plans orders and resident progress
Receive transcribe and implement physician orders accurately
Document nursing care and resident responses clearly and timely
Notify staff of new admissions room assignments and arrival times
Participate in facility inspections and surveys as required
Perform nursing administrative duties related to resident care
Assist with resident and family orientation to the facility
Conduct physician rounds and collaborate on resident care planning
Review resident charts for medications treatments and dietary orders
Prepare administer and document medications and treatments per physician orders
Ensure adequate supplies medications and equipment are available on the unit
Provide skilled nursing services including catheter care tube feedings wound care suctioning and ostomy care
Collect specimens (urine blood sputum) and perform routine lab-related tasks
Monitor and record vital signs and report changes in resident condition
Educate residents and family members on health care needs and treatments
Maintain confidentiality and comply with resident rights and HIPAA regulations
Perform additional duties as assigned to support facility operations
Requirements & Qualifications
Active Licensed Practical Nurse (LPN) license
BLS certification (required)
Long-term care or skilled nursing experience preferred
Strong documentation communication and organizational skills
