Job description:
Health Care Connectors is looking for compassionate and dedicatedRegistered Nurses (RN) with infusion backgroundto join our team in the Lower Fairfield County.
Qualifications:
- Minimal of two years of previous Home Health experience
- A current license as a Registered Nurse in Connecticut
- Have reliable transportation and be willing to commute to provide the best in-home patient care
Responsibilities:
- Perform assigned duties including administration of medication wound care treatments and procedures
- Monitor clients conditions; reporting changes to Nurse Manager
- Follow up with execute and properly document doctors orders
- Perform client assessments as necessary
- Collaborate with other professionals
- Manage home health aides or CNAs
- Weekend Rotation will be required. Typically this is one weekend per month or less.
