Registered Nurse (RN) Home Health PRN (Per Diem)

What Youll Do

• Perform start-of-care resumption recertification and routine skilled nursing visits

• Conduct comprehensive patient assessments (physical psychosocial and environmental)

• Develop and manage individualized plans of care in collaboration with physicians and the interdisciplinary team

• Provide patient and caregiver education on medications disease management and safety

• Monitor patient progress and communicate condition changes to the physician and clinical leadership

• Perform skilled nursing interventions (wound care medication management injections post-hospital follow-up etc.)

• Complete timely and accurate documentation (OASIS and visit notes)

• Coordinate services with therapists aides and other care team members

• Maintain patient privacy and confidentiality in compliance with HIPAA

What Youll Need

• Current unrestricted Registered Nurse (RN) license in the state

• Minimum 1 year of clinical nursing experience (home health strongly preferred)

• Strong assessment and patient education skills

• Reliable transportation valid drivers license and auto insurance

• Ability to travel within the assigned coverage area

• Comfortable working independently in a field-based setting

• Experience with OASIS and electronic documentation (HomeCare HomeBase a plus)
