SCOPE OF ROLE
Reporting to the Director Reproductive Health the Coordinator Social Services-Reproductive Health is responsible for providing and creating protocols related to counseling and social services pertaining to Signature Healths Sexual and Reproductive Health services as well as delivering services to patients requesting prenatal care or related sexual & reproductive healthcare.
HOW YOULL SUCCEED
- Provide guidance and education to individuals couples and/or families related to prenatal services postnatal services sexual assault/trauma and access to mental health and case management services.
- Provide education and consultative services to Signature Health employees as appropriate.
- As requested assist with reporting related to lake ADAMHS Board funded perinatal services.
- Provide the following services for prenatal clinic patients:
- Administer the Edinburgh Pre/Postnatal Depression Scale; interpret results and provide support and referral as appropriate
- Conduct psychosocial assessment
- Make referrals as necessary for mental and physical healthcare needs
- Provide referral to area social service/health resources
- Assist as appropriate with HCAP (Hospital Care Assurance Program) applications for continuity of care
- Assist with transfer to continuing prenatal care providers
- Comply with all agency policies and procedures.
- Communication/outreach with potential partnering providers to grow a community of referring providers
- Other duties as assigned.
KNOWLEDGE & EXPERIENCE
- Bachelors degree in social work or related field required.
- Valid unencumbered LSW or LPC License in the state of Ohio required.
- 3-5 years of experience working in a behavioral health role preferred.
- Demonstrated ability to make independent decisions assess organize prioritize and address multiple projects and problems concurrently.
- American Heart Association (AHA) Basic Life Support (BLS) certification required.
WORKING CONDITIONS
- Work is normally performed in a typical interior/office/clinical work environment.
- While hours of operation are generally standard flexibility to work evenings and extended hours may be required.
- Requires periods of sitting standing telephone and computer work.
- Hearing: adequate to hear clients or patients in person over the telephone or through telehealth technology.
- Speaking: adequate to speak to clients or patients in person over the telephone or through telehealth technology.
- Vision: Visual acuity adequate to perform job duties including reading information from printed sources and computer screens.
- Physical effort required: occasional lifting and carrying items weighing up to 15 pounds unassisted.
- Possible exposure to blood borne pathogens while performing job duties.
- Frequent bending reaching and repetitive hand movements standing walking squatting and sitting with some lifting pushing and pulling exerted regularly throughout a regular workday.
- Sufficient dexterity to operate a PC and other office equipment.
