Your role and responsibilities

You will:

• Work with a range of customers helping them maintain or regain their independence drawing on their strengths networks and the local community by using Signs of Safety (SOS) model.
• Play a critical role in the management of demand for adult social care including using digital solutions as tools for assessment and care
• Work in a fast-paced dynamic environment and will thrive in being part of a supportive team with a clear and shared vision.
• Carry out asset-based assessments/ reviews some of which are complex and challenging
• Carry out background research on referrals some of which will be distressing and emotionally/ mentally demanding.

The team

You will provide professional support to an area teams in a particular geographical area of Suffolk as we progress to the emerging new operating model and locality-based alliances. The area that you work in will have a number of area teams and will align to the clinical commissioning group footprint. You will support the Team Manager and the Senior Management Team who will be responsible for the operational delivery within it. You will form part of the Integrated Neighbourhood Teams and provide support and advice to frontline staff delivering social work services occupational therapy reablement services adult mental health and learning disability services. You will be expected to hold a small caseload to ensure that you maintain your professional focus.

You will work with local Contract and Service Development Teams and managers to understand develop and share good practice ensuring frontline teams share information to inform future commissioning models and new ways of working.
