The Opportunity
Belle is hiring a Director of Care Management to lead our centralized Care Management team (Case Managers and Medical Assistants). This team is the clinical engine behind our modelreviewing in-home visit findings and member data updating care plans coordinating care and ensuring members receive consistent follow-up month after month. This position reports to our Medical Director.
This leader will own day-to-day team performance and will be responsible for delivering a consistently high-quality compliant and scalable care management experience for Belle members.
This is a leadership role for someone who combines clinical credibility with strong operational instinctssomeone who can build structure coach teams improve workflows and operate with urgency.
