DescriptionJob SummaryThe Assessment Referral Coordinator supports patient care by coordinating referrals authorizations and follow-up appointments in collaboration with physicians and other care team members. This role ensures timely scheduling accurate documentation and clear communication with patients to facilitate continued access to services and high-quality care.
Essential Functions- Coordinates patient referrals authorizations and appointments for specialty care diagnostics and follow-up services in accordance with provider orders.
- Communicates with patients and families to explain next steps provide instructions and ensure understanding of referral and appointment requirements.
- Obtains prior authorizations and referral approvals as needed and accurately documents information in the electronic health record.
- Schedules appointments and follows up with external providers to confirm referral completion and collect consult notes.
- Responds to incoming calls and referral requests triaging appropriately and ensuring timely processing.
- Maintains accurate and organized patient records ensuring documentation is complete and submitted according to policy.
- Performs other duties as assigned.
- Maintains regular and reliable attendance.
- Complies with all policies and standards.
Qualifications- Associate Degree in Healthcare Administration or related field preferred
- 0-2 years of experience in a healthcare administrative or referral coordination role required
Knowledge Skills and Abilities- Knowledge of referral processes insurance requirements and prior authorization procedures.
- Proficiency with electronic health record systems and scheduling software.
- Excellent verbal and written communication skills.
- Strong attention to detail and organizational skills.
- Ability to manage multiple tasks in a fast-paced environment.
