Friendship Place offers the most effective model for addressing homelessness with innovative customized programs that have positive demonstrable results and a lasting impact on the community and beyond. Our programs are person-centric and individualized to meet the needs and goals of individuals families youth/young adults and Veterans experiencing homelessness. We provide outreach hospitality health care case management housing job placement services and advocacy. We have established a national presence and are known for sharing best practices in the field


Summary:

This position provides Homeless Prevention and Rapid Re-Housing services through the Supportive Services for Veteran Families Grant. Services are to be provided to low-income Veteran families that are at risk of becoming homeless or who currently are homeless. The Veterans First program follows a Housing First model and philosophy. The Case Manager will always utilize best practices and participant-centeredness. The Case Manager will assist Veteran Families in quickly maintaining or obtaining permanent housing with a goal of achieving housing stability within 90 days. The Case Manager will work to empower low-income Veteran Families in DC Arlington Alexandria Loudoun Prince William Fairfax Montgomery Frederick and Prince Georges counties to become stably housed.


Responsibilities:

  • Provide prevention and rapid re-housing services to at least 50 low-income Veteran Families annually
  • Maintain services to a caseload of a minimum of 15 Veteran families at any time
  • Develop implement and revise 90-day housing stability plan with Veteran Families.
  • Case Management as it pertains to housing stability plan
  • Deliver face-to-face contacts to participants when safe and appropriate
  • Foster wellness and independence
  • Engage participants and assess individual needs preferences and strengths
  • Assist participants in locating and maintaining permanent housing
  • Make appropriate referrals to identified needed services
  • Act as a liaison with landlords and other community stakeholders
  • Collaborate with agencies providing services to participants including community government and VA
  • Participate in multidisciplinary meetings and regular supervision
  • Maintain paper and electronic files
  • Maintain participants electronic records in the Homeless Management Information System
  • Demonstrate empathy and flexibility in responding to participants needs and requests
  • Attend professional trainings and conferences as required
  • Minimum of 50% of time spent in the field
  • Perform all other duties as assigned
Requirements

Education:

  • High School Diploma/GED Required
  • Bachelors Degree Preferred

Qualifications:

  • Prior relevant work experience in human or homeless services psychiatric rehabilitation addiction poverty foster care mental health youth/young adults and other individuals
  • Must have or be able to obtain a First Aid/CPR/AED certification that includes a hands-on skills testing module. Friendship Place will provide First Aid/CPR/AED training

  • Valid drivers license and ability to operate a motor vehicle.

Physical & Environmental Requirements: A minimum of 50% of time may be spent working in the field with participants in the car outdoors or in shelters. Other time will be spent in the office completing administrative and paperwork and meeting with the supervisor.


Background Checks

The selected candidate will be required to complete and pass background checks specific to contract requirements that may include reference checks education and employment history verifications criminal history background check and fingerprinting FBI Background Check National Sex Offender Registry Check and Driving Record check.

Specific Requirements: Must have own transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured registered etc.) and the employee must have a valid drivers license.

Compensation: The hiring hourly pay range is between $27.94 and $30.53. The starting pay is based on a step system. A candidates starting pay determination is made based on meeting the basic position requirements and current completed education credentials and work experience.
