Location: 1500 Market Street; Some travel throughout Eastern Pennsylvania

The Senior Program Manager will oversee program operations and implementation of the Technology Assistance Childrens Home Program (TACHP) and the Sickle Cell Disease Community-Based Services & Supports Program (SCD-CBSS) within the Family Services Department. The Senior Program Manager will supervise the Senior Program Coordinator for TACHP the Nurse Program Coordinator for SCD-CBSS and the Community Health Workers assigned to the Sickle Cell program. This position ensures effective program operations contractual compliance and collaborative partnerships across healthcare community and statewide networks. The Senior Program Manager will support program development foster relationships with funders and partners participate in statewide meetings lead quality improvement efforts and maintain Apricot database oversight for program reporting and evaluation. The Senior Program Manager will additionally serve as a SafeCare Coach to provide coaching and fidelity support to SafeCare Providers. This position requires travel throughout Pennsylvania to attend leadership funder and statewide meetings as needed. Must clear child abuse criminal history check and FBI clearance.

Responsibilities:

  • Provides leadership and oversight of TACHP and SCD-CBSS program activities.
  • Works with the Assistant Director Family Services to coordinate and implement contractual and grant-funded obligations and ensure timely program deliverables.
  • Establishes and maintains strong relationships with funders partners providers and project staff to ensure effective communication and efficient program implementation.
  • Supports evaluation efforts with PHMCs Research and Evaluation Group including contributing expertise to survey and data-collection method development and providing data and information upon request.
  • Participates in quarterly association and advisory meetings in various counties of PA.
  • Collaborates with medical staff/providers advisory committees and referral sources to assess and determine the most appropriate practices resources and service pathways.
  • Ensures accurate and timely program reporting and documentation; oversees the quality and compliance of the Apricot database.
  • Works with the Assistant Director to ensure goals and deadlines are met.
  • Supports completion of program reports with outcome measures and other written documentation.
  • Maintains accurate records according to grant funding and observes HIPAA requirements; generates data and reports appropriately.
  • Attends meetings and conferences in Eastern PA aimed at developing the capacity to serve technology-assisted children.
  • Keeps abreast of up-to-date advancements regarding medically complex children SCD and evidence-informed practices and shares information and resources with staff as appropriate.
  • Reviews and integrates new ideas and concepts with Assistant Director in order to improve project delivery content and/or evaluation for the target audience.
  • Participates in conversations aligned with HPCs strategic plan in regularly scheduled meetings. Participates in grant writing tasks as needed by upper management to develop concepts and to write proposals to grow organizational initiatives; contributes content expertise for grant proposals as needed.
  • Ensures educational materials are culturally appropriate for diverse populations especially for those with limited reading and English skills.
  • Participates in HPC management team meetings.
  • Represents TACHP SCD-CBSS and HPC at various meetings.
  • Must be willing to travel throughout Eastern Pennsylvania.
  • Ability to acquire information about new systems organizations and practices.

Safe Care Coaching

  • Support and provide coaching on the Safe Care program an evidence-based curriculum to prevent child abuse and neglect in children under the age of 5.
  • Conduct ongoing fidelity assessments and coaching sessions to support Provider certification as needed.

Supervision/Leadership

  • Conducts regular supervision of the Senior Program Coordinator (TACHP) Nurse Program Coordinator (SCDâCBSS) and Community Health Workers assigned to Sickle Cell.
    • Provides supervisory oversight to ensure Program Coordinators meet programmatic contractual and MCO-related documentation and reporting requirements.
  • Assists staff in the use of program materials written forms assessments case notes referral forms discharge plans and other written documentation and entry of these forms into the online database.
  • Supports staff in defining priorities deadlines growth opportunities and addressing program challenges.
  • Assists with program hiring and oversees training and orientation of new program staff.
  • Provides mentoring to staff to foster professional and personal growth.
  • Participates in case reviews staff meetings and professional development activities as required.
  • Establishes and maintains linkages with medical providers community health social service education system legal service agencies and community partners.
  • Writes and conducts annual staff performance management reviews for supervised staff.
  • Communicates formally on a weekly basis with the Assistant Director.
  • Enforces agency policies and procedures.
  • Other relevant duties as assigned.

Skills:

  • Must demonstrate strong organization time management and problem-solving skills.
  • Excellent problem-solving conflict resolution time management and professional communication (written and oral) skills.
  • Ability to acquire information about statewide systems organizations and practices.
  • Ability to establish priorities and work both independently and in a team environment to meet objectives with minimal supervision.
  • Advanced proficiency in the Microsoft Office suite and various web-based platforms with an ability to learn new software as needed.
  • Experience using motivational interviewing techniques.

Experience:

  • Four years of related supervisory experience is required.

Preferred

  • Home visiting experience
  • Knowledge of navigating medical and social service resources for medically fragile children and technology-assisted children.
  • Knowledge of public health theories principles and practices.
  • Experience in data collection/entry evaluation monitoring and needs assessments.
  • Experience in monitoring budgets.
  • Experience with grant writing for public health or a related field.
  • Experience working with diverse populations and low-income individuals.
  • Demonstrated experience working with health care providers and knowledge of issues related to maternal and child health child development child abuse prevention and children with special health care needs.

Education Requirement:

A bachelors degree in social work or a related field is required.

A Masters degree in social work or a related field is preferred.
