Bilingual Spanish Case Assistant
Working for a non for profit organization located in NYC.
Conduct community outreach to raise awareness about services available under the NYS 1115 Medicaid Waiver.
Engage individuals families and community organizations to promote participation ineligible programs.
Assist individuals in understanding eligibility criteria and screening potential applicants for the 1115 Waiver program.
Support clients by completing enrollment applications and gathering required documentation.
Assess clients needs related to medical behavioral health and social care (e.g. housing food
access transportation) and connect clients to appropriate clinical providers health plans and community support.
Coordinate follow-ups with clients and/or resources providers to ensure clients receive scheduled services and resolve barriers.
Maintain accurate case records and client progress notes in various databases
Participate in training and meetings as required
Qualifications
Education and/or experience required:
Bachelors degree requires 3-5 years of experience in a social service agency providing case management support
