Join us as a Transition Facilitator and be part of something bigger.

What youll do

We are seeking a Transition Facilitator to join The Arches team within the Bail Accommodation Support Program (BASP). Participants at The Arches come from diverse backgrounds and often experience multiple and complex challenges and involvement within the criminal justice system.

In this role you will work alongside participants using a person-centred strengths-based approach to support their transition from short-term bail accommodation into stable longer-term housing. You will also assist participants to build community connections and access the supports they need to sustain their accommodation and wellbeing.

This is a full-time maternity leave contract until 26th February 2027 based at Port Adelaide.

Youll be trusted to make a valuable impact by:

Providing case management support to facilitate the transition of participants to suitable longer term accommodation

Supporting personal wellbeing through tailored support services addressing various needs such as: housing & accommodation employment financial stability social connections & more.

Working collaboratively with internal and external specialists and service providers to better meet participants needs.

Identifying risks and barriers to housing and work proactively to address them.

What youll bring

To thrive in this role you will have:

Tertiary qualifications in Social Work Community Services or a related field or significant relevant experience.

Knowledge of the community services sector in South Australia with a specific emphasis on homelessnessand the criminal justice sectors.

Demonstrated experience supporting people who are homeless or at risk of homelessness.

High level of cultural awareness and commitment to working respectfully with Aboriginal communities.

We would also love you to have:

Experience working with individuals with complex needs including those involved in the criminal justice system.
