Job Description:
We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to join our team in Farmington NM. This position focuses on providing high-quality nursing care to patients in their homes while maintaining accurate and timely documentation.

Requirements:

Active RN license in the state of New Mexico
Home health experience preferred
Strong clinical assessment and communication skills
Ability to work independently
Experience with Matrix charting is a plus

Responsibilities:

Provide skilled nursing care to home health patients
Perform patient assessments and develop care plans
Administer medications and treatments as ordered
Educate patients and families on care and disease management
Complete clinical documentation using Matrix charting system
Coordinate care with physicians and interdisciplinary team members
