CERTIFICATION REQUIREMENTS :
ADDITIONAL LICENSE REQUIREMENTS : Must have active NBSTSA at time of submission
Weekend Requirements : Weekend schedule to be determined by facility
On Call Requirements : On call schedule to be determined by facility
This form needs to be 100% accurate and have all request noted in the highlights section.
- Start Date should be on or very quickly after the Order Start Date listed.
- All RTO should be listed on the cover page must be less than 7 days.
- RTO will not be approved in the first week of assignment or holidays.
- All facilities the traveler is open to working in should be listed on the submission.
- If interested in any location please include what states (I.E. Any AZ Any CO Any state etc.)
- All shifts the traveler is open to working on should be listed on the submission.
- All departments the traveler is open to working must be checked on the cover page.
- All candidates submitted should be preclosed and ready to accept.
- Please let us know if the traveler withdraws or is no longer available.
Requirement description :
- Provider must have 24 months minimum of current Certified Surgical Tech/CST experience - REQUIRED
- Certifications must be American Heart Association or American Red Cross - REQUIRED
- Verification cannot show any hits on license at time of submission - REQUIRED DOCUMENT
- Banner will not approve RTO within the first week or holidays. Can not exceed 7 days - REQUIRED
- Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**

- Flu vaccination - REQUIRED
- Drivers License - REQUIRED
