What Youll Do

As a Registered Nurse Home Health you will:

• Deliver skilled nursing care aligned with Elara Carings mission and clinical standards

• Manage a caseload by assessing planning coordinating implementing and evaluating patient care

• Complete initial and ongoing patient assessments identifying physical psychosocial environmental and spiritual needs

• Develop and revise individualized plans of care based on patient needs and progress

• Communicate significant findings and changes in condition to physicians Clinical Supervisors and interdisciplinary team members

• Conduct follow-up visits based on patient family and facility needs

• Ensure patient and staff privacy and confidentiality in compliance with HIPAA

• Collaborate closely with caregivers families and the interdisciplinary care team

What is Required

• Current unrestricted RN license in the state

• At least one year of nursing experience in a clinical care setting

• Ability and willingness to travel within the branch coverage area (up to 50%)

• Dependable vehicle valid drivers license and current auto insurance

• Associate or Bachelors Degree in Nursing preferred

• Home Health or Hospice experience highly preferred

• Experience with HomeCare HomeBase preferred

• Ability to safely push pull lift and move up to 50 lbs to 100 lbs
