General Summary
The Value-Based Care LPN Outreach Coordinator supports MVHCs population health and value-based care initiatives by conducting proactive outreach to patients closing care gaps and coordinating services with our RN care management team that improve quality outcomes. This role works closely with care managers and quality teams to engage patients in preventive care chronic disease management and post-discharge follow-up.
Minimum Qualifications:
- High school diploma
- Completion of an accredited educational program for Practical Nurse and current non-restricted Ohio LPN license.
- Current CPR certification required
- Up to date immunizations as required by MVHC
- Good written and verbal communication skills
- Good computer skills; proficiency with office equipment personal computers and word processing
- Ability to multi-task
- Strong critical thinking skills and accuracy
- Knowledge of general office procedures and practices
Working Conditions:
- Lifting up to 20 lbs occasionally
- Carry up to 20 lbs occasionally
- Sitting 1-2 hours at a time
- Must be able to operate a multi-line telephone
- Must be able to read write and understand English well
- Must have skills and knowledge of software as designated by practice
- Withstanding pressure and deal with emergency situations as needed
- Average working hours with possible non-traditional hours
- Ability to deal with upset or disgruntle patients in order to deescalate the situation
- Normal coordination and/or visual and aural attention in connection with the standard flow of work
- Repetitive movements of the hands wrists and fingers; good visual acuity (with or without correction)
- Good working conditions in an office environment
- Manual dexterity and ability to sit for long periods are required
- Listening skills sustained concentration and efficiency under difficult situations are at times required to maintain patient flow
Duties and Responsibilities:
- Develops and maintains patient relationship by making patient feel at ease
- Conduct outbound and inbound patient outreach via phone portal and other communication methods to support value-based care programs
- Identify and address care gaps related to preventive screenings chronic conditions and quality measures (e.g. HEDIS CMS ACO metrics)
- Educate patients on recommended care medication adherence and follow-up appointments
- Coordinate care with providers care managers social workers and community resources
- Perform post-discharge and transitional care follow-ups as assigned
- Document outreach activities assessments and interventions accurately in the EHR
- Support quality improvement initiatives and audits
- Maintain compliance with HIPAA and organizational policies
- Maintains strict patient confidentiality at all times
- Manages all communication in Artera (text messaging platform)
- Schedule patients for appointments
- Responds efficiently and accurately to patient calls explaining possible solutions and ensuring that patients feel supported and valued
- Engage in active listening with callers confirming or clarifying information
- Conducts inpatient follow up calls scheduling the appropriate follow up appointment(s)
- Provides assistance to patients as needed
- Communicates effectively with patient in an effort to gather necessary information
- Maintains professional and relationship building etiquette when handling customers third party payers hospitals and other outside contracts
- Follows rules and procedures for routine tasks
- Handle difficult situations appropriately
- Performs data entry
- Remains pleasant polite and efficient while handling difficult situations
- Accepts constructive feedback from supervisors and works to improve performance
- Exhibits pride in the job and with Muskingum Valley Health Centers mission and values
- Maintains the privacy of patient information and the security of all records
- Concentrates and works effectively despite distractions
- Adheres to all MVHCs policies and procedures fully cooperating with the MVHC Compliance Program
