Job Summary

We are seeking a dedicated and compassionate Psychiatric Nurse Practitioner to join our Community Adult Assertive Community Treatment (ACT) team in Queens County. The Psychiatric Nurse Practitioner will provide psychiatric evaluation diagnosis treatment planning and medication management for individuals with serious mental illness and co-occurring disorders in a mobile community-based setting. This role emphasizes close collaboration with and support of a multidisciplinary team to deliver flexible person-centered recovery-oriented care. Every program participant is expected to meet with the psychiatric provider at least once monthly. These services must be provided in the community based on the need of the individual participant.

PRIMARY FUNCTIONS


  • Provide psychiatric evaluations diagnoses and treatment for individuals served by the ACT team.
  • Prescribe and manage psychiatric medications in alignment with evidence-based practices and ACT standards.
  • Conduct regular follow-up appointments to monitor progress and adjust treatment plans.
  • Collaborate daily with ACT team members (nurses social workers substance use specialists vocational specialists and peer staff) to ensure integrated coordinated care.
  • Participate in team meetings treatment planning and case consultations.
  • Offer psychiatric expertise and consultation to ACT staff on complex clinical presentations and support and advise the team on interventions and treatment planning.
  • Support crisis intervention and stabilization in coordination with team members
  • Ensure timely and accurate documentation in accordance with OMH regulations ACT fidelity standards and agency policies.
  • Maintain current knowledge of advances in psychiatry behavioral health care and best practices in ACT service delivery.

ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT


  • Contribute to quality improvement initiatives and outcome measurement within the ACT program.
  • Participate in trainings and professional development to support high-quality ACT services.
  • Perform additional duties as assigned to support team and agency needs.

QUALIFICATIONS AND ATTRIBUTES


  • Strong knowledge of psychiatric diagnosis treatment and recovery-oriented care for individuals with serious mental illness including co-occurring disorders.
  • Knowledge of federal and state regulations related to community mental health services.
  • Commitment to culturally competent trauma-informed and recovery-focused practice.
  • Ability to function effectively within a multidisciplinary team.
  • Strong communication and interpersonal skills with the ability to engage individuals in community-based settings.
  • Demonstrate high levels of integrity professionalism and ethical standards.
  • Proficiency with Electronic Medical Record (EMR) systems; basic Microsoft Office skills preferred.

EDUCATION AND EXPERIENCE


  • Board-certified Psychiatric Nurse Practitioner with appropriate experience or a strong desire to work in this setting
  • Experience with ACT community-based psychiatry or treatment of serious mental illness preferred.
  • Six months of experience with serious mental illness required
  • Flexibility in scheduling and willingness to provide services in both office and community settings is essential.
