MAIN FUNCTIONS: The Valley Council for Health and Human Services (VC) is a cross-sector group of organizations working together to improve the health and quality of life of Connecticuts Lower Naugatuck Valley residents by identifying community needs and developing culturally responsive services delivered by our membership and partners. This is a position with the Valley Council For Health and Human Services not Griffin Health.
VC is hiring a Project Coordinator who will be responsible for supporting VCs efforts to foster coordination and collaboration among the regions health and human services agencies.
The Project Coordinator will be responsible for:
1. Monthly VC Meetings
a. Attend monthly meeting (first Tuesday of month 8:30-9:45)
b. Take attendance during meeting
c. Take minutes during meeting
d. Write up minutes for distribution within 48 hours of meeting
e. Track meeting attendance on database
f. Develop presentations as needed
g. Coordinate meeting presenters
i. Schedule presenters
ii. Confirm attendance
iii. Obtain presentations prior to meetings
2. Quarterly Retreats
a. Assist in planning logistics
b. Assist in preparing materials/packets/etc
c. Attend retreat
d. Take attendance and minutes during retreat
e. Assist in facilitating group activities
f. Write up minutes for distribution within 48 hours of retreat
g. Track retreat attendance
3. Membership
a. Identify and contact potential new member organizations
b. Develop database of active member organizations with contact information
c. Maintain Listserv to be comprehensive including addition of organizations not currently members
d. Onboard new members
i. Add to Listserv
ii. Send membership form
iii. Send dues information or offer trial 1 year membership
iv. Obtain completed membership form
v. Obtain dues payment (may be after member trial period)
vi. Add to member database
4. Annual Membership/Dues Drive
a. Manage annual drive including material development and communication with member organizations
b. Follow-up to obtain dues and completed membership forms
c. Maintain membership tracking document and update member database
5. Communication
a. Website
i. Manage website and maintain current information
ii. Add new tabs/information to website as needed
iii. Maintain current member list on website and hyperlink
iv. Develop QR flyer/postcard to link to member list and resources
v. Annually evaluate and refresh website
b. Social Media
i. Develop social media strategy and implement
ii. Maintain Facebook account
iii. Establish other social media accounts and maintain
iv. At least weekly post and/or share member posts
c. VC Information
i. Develop VC informational packet for potential members
ii. Distribute packet as needed
iii. Annually refresh packet
6. Task Forces and Committees
a. Provide support to task force leaders
b. Attend committee meetings
c. Schedule task force and committee meetings
d. Collect and distribute task minutes
e. Take and distribute committee minutes within 48 hours of meetings
f. Support task force and committee initiatives
7. Community Care Hub
a. Develop and manage Unite Us dashboard
b. Develop reporting templates (or oversee intern project)
c. Prepare monthly reports
d. Analyze data
e. Provide information and reports as needed
8. Grant Writing
a. Assist in developing base grant application
b. Coordinate member profile from database
c. Gather data as needed
9. Valley Community Index
a. Participate in development of strategy
b. Coordinate meetings
c. Support committees as needed
10. Manage other projects and initiatives as to be determined that support the mission of VC.
The Project Coordinator will deliver on these responsibilities by supporting the Executive Director and working collaboratively with internal and external stakeholders in the Valleys health and human services system. The Project Coordinator will engage with the full Council on an on-going basis. This position will be employed by Griffin Health. The Director will report directly to the Executive Director.
