Licensed Mental Health Counselor Associate (LMHC-A) or Licensed Social Worker (LSW) Open Door Health Services Integrated Behavioral Health Overview
- Open Door Health Services is seeking a compassionate and motivated LMHC-A or LSW to join our integrated behavioral health team. Our behavioral health services are embedded within a large primary care practice that has served the community for nearly 30 years. This role is ideal for a clinician who values collaboration continuity of care and making a meaningful impact in the lives of patients. We are especially interested in candidates who are looking for a long-term professional home and who are committed to growing their clinical skills while investing in the community we serve.
What You Will Do - Provide individual and group counseling services to patients with a wide range of behavioral health needs including anxiety depression trauma and substance use concerns
- Conduct comprehensive assessments and develop individualized treatment plans based on patient needs and goals
- Deliver patient-centered strengths-based therapy in a supportive and non-judgmental environment
- Maintain timely accurate and confidential clinical documentation in compliance with HIPAA and organizational standards
- Monitor patient progress and adjust treatment plans as clinically appropriate
- Provide education and referrals to community resources and therapeutic supports
- Collaborate closely with primary care providers families and community partners to ensure coordinated and integrated care
- Participate in team meetings case consultations and supervision
- Engage in ongoing professional development and training opportunities

What You Will Need - Education - Masters degree in Social Work Counseling or a related field required
- Licensure - One of the following Indiana licenses required prior to hire:
- Licensed Mental Health Counselor Associate (LMHC-A)
- Licensed Social Worker (LSW)
Additional Qualifications - Knowledge of clinical documentation and diagnostic standards (ICD)
- Familiarity with HIPAA regulations
- Strong interpersonal communication and organizational skills
- Ability to work effectively within a team-based integrated care setting
- Commitment to ethical practice inclusivity and patient-centered care
- BLS certification preferred (American Heart Association or American Red Cross)
