Job Summary

Department Overview

The Social Work Specialist works in collaboration or by delegation from CSWA or LCSW to screen patient/family for psycho-social needs and determines current and anticipated patient care concerns. The SW Specialist advocates for patients and families to connect with community resources. The SW Specialist provides families with information regarding patient care issues; facilitates in the coordination of follow up care in OHSU clinics or in the community; participates in patient and familys health care education assists with housing and transportation needs as necessary.

This position is a flexible work position which allows for some or complete remote work. Remote work at OHSU is a work arrangement under which an employee performs the duties and responsibilities of their position and other authorized activities from a non-OHSU location as part of their approved regular work schedule. The Social Work Specialist should have a private space at home dedicated to work and no Protected Health Information should be printed.

Function/Duties of Position

  • The Social Work specialist connects families to resources as determined by clinic team. The SW Specialist screens patients and families ability to cope with major life changes provides support to patients and families and refers to a Medical Social Worker as necessary. The SW Specialist provides families with information regarding patient care issues; facilitates in the coordination of follow up care in OHSU clinics or in the community; participates in patient and familys health care education assists with housing and transportation needs as necessary.

  • Documentation: Maintains accurate and timely documentation of patient care services including charting reports and documentation. Submits patient care statistics in a timely manner.

  • Information Syestems: Creates and assists in form development as needed; utilizes data base access system for information relevant to patients care.

  • Professional Development: Participates in department meetings; participates in workshops seminars and training sessions to maintain professional competency. Participates in patient rounds as appropriate. Assists in staff education. Visits community facilities programs or services to acquire pertinent resource information.

  • Maintains accurate and timely documentation of patient care activities an efficent manner per departmental policies. Participates in Department of Care Management and assigned service meetings in services and conferences as indicated. Meets in regularly scheduled supervisory sessions. Complete all OHSU/JCAHO mandatory education requirements and activities within established time lines.

  • Yearly competencies as identified by department

Required Qualifications

  • Two years of social service work experience providing supportive counseling to clients on a one-to-one basis which includes full responsibility for handling each case plan; AND

  • A Bachelors degree or three more years of experience which shows:

    • a) knowledge of casework methods and techniques;

    • b) knowledge of patients familys rights;

    • c) knowledge of medical terminology; and

    • d)knowledge of implications of the effect of illness injury and/or disability on patients and families.

Preferred Qualifications

  • Bachelors degree in Social Work or related field preferred
  • 3 or more years of hospital experience
