How youll contribute

APatient Care Coordinatorwho excels in this role:

  • Coordinate care and communication with internal and external providers including medical psychiatric family school social services and community agencies.
  • Provide timely clinical updates to the treatment team based on information from families referral sources and community partners.
  • Maintain proactive compliant communication with patients families and referral sources in accordance with HIPAA requirements.
  • Provide postdischarge followup build rapport mentor patients and encourage continued engagement in care.
  • Execute structured postdischarge followup protocols including early transition calls scheduled outpatient coordination ongoing checkins and crisis response support.
  • Participate in crisis intervention and deescalation efforts to ensure patient and staff safety when needed.
  • Assist patients with social determinants of health needs including housing transportation insurance and access to community resources.
  • Facilitate family engagement including providing family sessions when clinically appropriate to support communication and treatment involvement.
  • Complete required clinical assessments and documentation including psychosocial evaluations levels of care updates safety screenings and assigned tools.
  • Actively contribute to treatment planning psychosocial updates and discharge/transition planning processes.
  • Provide patient and family education regarding treatment services program expectations and the continuum of care.
  • Support transitions of care by providing referrals resource education and warm handoffs to outpatient and communitybased providers.
  • Collaborate effectively with the interdisciplinary team including physicians therapists utilization review nursing and clinical services.
  • Communicate treatment progress and lengthofstay considerations to utilization review and assist with authorization or denial processes as requested.
  • Complete all patient and hospital documentation accurately timely and in compliance with regulatory accreditation and organizational standards.

What were looking for

  • Education: Masters degree in Social Work Counseling or related field from an accredited school required.
  • License:Counseling or Social Work license required.
  • Minimum of one year of experience in a psychiatric or mental health setting preferred.
  • CPR certification is required within 30 days of employment and prior to any patient contact.
  • De-escalation certification is required within 30 days of employment and prior to any patient contact.
