POSITION TITLE: Case Manager Jail Continuity of Care

SALARY: $3155-$3255 Salary/monthly

GENERAL DESCRIPTION:

Under the supervision of the Director of Jail Based Services the Jail CoC Case manager will screen administer assessments and provide referrals to other MHMRCV programs and appropriate community resources prior to the inmates release. The candidate will meet with each program participant weekly. The candidate will work closely with jail staff and community organizations. This position will be housed at the Tom Green County Jail. The goal is to link criminal justice involved individuals with appropriate mental health substance use disorder and other needed services in order to reduce recidivism.

The ideal candidate possesses a strong knowledge of mental health and the environment of corrections. This position requires the ability to communicate effectively with diverse cultures and socio-economic populations. The role uses skills and experience in working with integrated computer database programs to access files and document treatment services and the ability to organize and prioritize work assignments. This position is the best fit for a person able to work both independently and collaboratively. This position supports other crisis programs including Crisis Diversion and Crisis Respite Unit as needed.

EDUCATION:

  • Bachelor or advanced Degree from an accredited college or university with a major in social behavioral or human service field including psychology social work sociology human development criminal justice. LBSW AND LMSW ENCOURAGED TO APPLY

LICENSE/CERTIFICATION:

  • Valid Texas drivers license and must be insurable on the Centers insurance. No more than 3 moving violations in the last 3 years.

EXPERIENCE:

  • Experience in mental health and/or criminal justice and knowledge about community resources and social services is preferred.
  • Preferred - Clinical and administrative experience working with individuals with a mental illness and/or co-occurring substance abuse disorder in an emergency setting; bilingual (English/Spanish).

KNOWLEDGE SKILLS AND ABILITIES:

  • Ability to work in a rapid pace environment in an organized and professional manner.
  • Ability to self-monitor production and quality with minimal assistance.
  • Proficient in the use of electronic data systems including electronic health records and business software such as Microsoft Word and Excel.
  • Ability to develop productive relationships with individuals and potentially family members that focus on individual specific outcomes.
  • Ability to recognize barriers and coordination issues between service systems and to advocate effectively for individuals served.
  • Organization and typing skills are a must.
  • The ability to work as a team member in a diverse environment interfacing with both internal and external customers.
  • Experience with tracking data and timely data reporting is preferred.

Shift: Monday Friday 8 a.m.-5 p.m. Some evening hours.
