The Guild utilizes the collaborative efforts of leading professionals best-practice approaches tailored curricula and unmatched care to treat the individuals we : The mission of The Guild for Human Services is to educate encourage and empower individuals with intellectual disabilities so they may achieve their full potential to lead high-quality lives and participate meaningfully in the : Nurse Case Manager acts as a committed member of a multi-disciplinary team to help create and maintain goals of health & wellness for an assigned group of Job Responsibilities:The Nurse Case Manager/ Registered Nurse acts as Nurse Consultant for assigned Group Home the health and safety issues within all locations of the school and staff training to ensure optimal health and safety staff training pertinent to health and safety needs medical conditions medical diagnosis and treatment in scheduling laboratory testing and medical appointments and participates in health examinations as medical records including documentation of treatment and medication administration of with outside agencies and physicians as initial and annual medical assessments for annual Individual Health Plans and quarterly plan for and communicate with the team regarding medical appointments medical issues and other health related with outside agencies physicians and parents/ in designated team meetings and case reviews for Medication systems in residences and timely submission of Medication Error certified staff medication administration competenceProvide medical on-call coverage at minimum one week monthlyAble to attend training as requiredOther related duties as assignedEssential Job FunctionsRegular attendance at work is an essential function of the job including inclement weather situationsPhysical and mental capacity to work in stressful situations and de-escalate individuals who have limited cognition and complex needsProficiency in written and spoken English is an essential function of the organization and time management skillsPass Guild Driving test to access Guild vehicles for transportations of be able to implement crisis intervention techniques as necessary including Nonviolent Crisis Intervention (via certification through CPI)
