Intake Coordinator
Location:In office Fort Collins CO 80525
Hourly rates:$18 - $20
Status: Full-time
Schedule: Monday to Friday 8-5
Interim HealthCare is looking for anIntake Coordinator. We are a health care company that has been in business locally for 30 years and is still growing strong! This is a key role in our organization and we are looking for someone who can make a great first impression and can follow up with excellent customer service. This is a great opportunity to join a home health team of a long-standing stable and reputable company.
Responsibilities Include (but not limited to):
- Acts as a contact telephone liaison person interfacing with referral sources outside agencies health professionals and field staff.
- Receives and coordinates all incoming calls providing patients/clients and referral sources with basic information to assist in accessing appropriate services
- Communicates with patient/client and family regarding arrangements for the initiation of care/services.
- Completes intake screening including obtaining documenting and analyzing all required information.
- Receives and logs referrals. Prepares reports and responses to inquiries. Provides telephonic contact and system data entry for customers seeking care/services.
- Collects and enters customer information into the management information system.
- Participates in on-call rotation including 1 weekend per month.
- Completes other assignments as requested and assigned.
Job Requirements
- Previous home care / health care / medical office experience preferred including experience and knowledge of insurance authorizations admissions or intake processes.
- Experience interfacing with referral sources outside agencies health professionals and field staff.
- Knowledge of medical terminology.
- Strong organizations skills: ability to manage multiple tasks prioritize and work independently in a fast paced high-volume environment and focus on details.
- Excellent customer service phone computer and data entry skills.
- Computer proficiency to include current company software programs.
- Ability to develop strong working relationships and works well with a team.
- Pass a background check and a drug test(we are required to test for THC)
