Level 1 (Entry level up to 2 years of direct experience):$24.04 per hour
Level II (3 years of direct experience):$25.19 per hour
Bilingual premium for qualifying languages is 3.5% higher than starting salary ifpassbilingual exam on first week of hire.

Position Summary:
Working under the Community Engagement Department (including Healthy Aging Programs) and in collaboration with CHN Client Services (including Case Management and Housing) this full-time positionsupports people living with HIV (PLWH) residing at Daves Place and Juan Diego by addressing their unique needs through comprehensive case management care coordination housingassistance and Place is a program working in conjunction with CHNs Housing and Healthy Aging Programs serving older adults living with HIV in a community-based residential setting with supportive services offered Diego is a Permanent Supportive Housing Program within the Coordinated Entry System forPLWHexiting chronic homelessness. The Residential Services Coordinator will ensure program participants housing stability health and independence through medical case management resource navigation geriatric-focusedinterventionsand a collaborative care approach in alignment with theiCHANGEand VITAL-AGE models (when applicable). The position plays a key role in implementing an aging-in-place model tailored to older PLWH at Daves Place and a Housing First model at Juan Diego fostering long-term housing stability addressing health disparities and enhancing the quality of life for residents at both properties.
Essential Job Functions:
Case Management Duties:
Conduct comprehensive intake and needs assessments toidentifyclient strengths and needs including but not limited to connection to healthcare medication adherence income and employment housing needs mental health and substance abuse issues food and nutrition and financial and transportation needs.
Develop strategies and goals through Individual Wellness Plans and Intensive Case Management Plans in collaboration with clients to meet identified needs.
Tailor Individual Wellness Plans and Intensive Case Management plans to clients aging and health-related goals followinggeriatric care model protocol when applicable.
Provide ongoing case managementmonitoringclient progress and revising care plans to address evolving needs coordinating care among interdisciplinary teams including medical providers behavioral health staff and external aging service agencies.
Support participants in accessing healthcare public benefits (e.g. Medicare/Medicaid) nutrition services and caregiver support as needed.
Collaborate with the property management company to address tenancy issues as they arise.
Collaborate with the Housing Occupancy Coordinator to ensure that all clientsrecertifytheir housing subsidies on an annual basis.
Assistwith caseload coverage for other Medical Case Managers at DCHN as needed.
Data Management and Reporting
Maintainaccuratedocumentation of client screening andcareIWP data interactions services provided and program outcomes.
Collect and analyze data for grant reporting billing and program evaluations to ensure alignment with aging program goals.
Use data insights to recommend improvements to aging-in-place models and participant care strategies.
Program Development and Community Engagement
Collaborate with the Housing and Healthy Aging Program teams to deliver services aligned with program models specifically the VITAL-AGE aging-in-place model.
Facilitate client engagement in wellness activities social circles and educational programs to enhance quality of life and independence.
Community Advocacy and Resource Development
Strengthen partnerships with community organizations and service providers to expand aging-related resources for older PLWH tenants.
Act as an advocate for participants aging and health needs in the community addressing gaps in services and resources.
Develop culturally competent outreach initiatives to engage marginalized populations including LGBTQ and BIPOC clients.
Must be able to work in a diverse setting with diverse populations including persons living with HIV the LGBTQ community persons of various ethnic backgrounds disenfranchised communities.
Participation in training related to HIV substance use harm reduction trauma informedcareand related issues. Cultural Competency/Sensitivity.

Experience working in or with senior housing long-term care assisted living or other supportive housing addition 2 years of case management experience and training or 3 yearsof Promotora experience is with group facilitation and mediation is be able to work in a diverse setting with diverse populations including sexual orientation. This role is ideal for a professional with relevant knowledge in the fields of aging and HIV/AIDS as well as experience in providing Care Management services. The candidate should have a passion for working with older adults and individuals living with HIV/AIDSpossessstrong problem-solving skills andmaintainan optimistic outlookregardingthe possibilities for our client Spanish and multicultural experience are strongly preferred.
Education:
Bachelors degree in social work Human Services Psychology Sociology Public Health Gerontology and/orequivalentcombination of education andexperiencerequired; candidates with lived experience can substitute educational background. Candidates who are actively pursuing more education or have hopes to pursueadditionaleducation are encouraged to apply. Candidates with lived experience can substitute educational experience.
Software Utilized:
Proficiencywith Microsoft Office applications such as Word Excel Outlook and Teams isrequired. Candidates should be comfortable working with client database systems and learning new software tools as needed.
KnowledgeSkilland Ability:
Previousexperience in the delivery ofevidenced-based programs is highly preferred as well as excellent communication client services and data collection skills. Theappropriate personwill be highly organized anddetail-oriented. This position requires personal and professional accountability; self-management and initiative; dedication to quality improvement; professional integrity; research ethics and cultural competency. Mustdemonstrateaccuracyefficiencyand superior organizational skills. Mustdemonstrateeffectiveness in the following areas: task and time management; teamwork; and communication with teams managers clients and participants and company consultants.

Physical Demands and Work Conditions:
Works in an office environment. May travel via automobile or airplane to offsite locations or for CHN business. Communicates with clients and vendors using the telephone e-mail the Internet and clear speaking ability and the ability to use a telephone for extended periods. Requiresabilityto view a computer screen (near acuity). Requires fingering agility for ability to sit stand and engage in extended periods of screen time or communication by phone is necessary to perform the essential duties of the role.
