CASE MANAGER SPECIAL CHILD HEALTH SERVICES CASE MANAGEMENT PROGRAM (GRANT)

12 to 16hours per week as needed $45-$55/hr. depending on experience

QUALIFICATIONS

The position of the Case Manager may be filled by either (a) a New Jersey licensed Registered Nurse with a Bachelors Degree with two year of experience in pediatrics and/or public health or (b) a Licensed Social Worker with two years experience in pediatrics and/or public health or Licensed Psychologist. This position requires the medical and physical capacity to perform the major responsibilities.

APPOINTMENT

Appointment shall be made by the Board of Education upon the recommendation of the Superintendent of Schools.

CONTRACT AND SALARY

The salary of the position for the Case Manager shall be established by the Board of Education as hourly as needed and dependent upon experience.

REPORTS TO

The Case Manager shall report to the Supervisor of Special Child Health Services Case Management Program. His/her performance shall be evaluated in a manner consistent with policies and procedures of the Board of Education.

GENERAL FUNCTION

A Case Manager actively works with the child/parent as the unifying agent available and accessible to the child/parent. He/she helps the child/parent through the maze of the human service delivery program.

RESPONSIBILITIES

In order to fulfill his/her responsibilities the case manager shall perform but not limited to the following duties:

  • Counsel the child/parent including:
  • a description of the Special Child Health Services Case Management Program;
  • identifying the childs problem(s) with the parents;
  • validating the parents knowledge and understanding of the childs problem(s).
  • Assess the need for services and developing with the parent an Individual Service Plan.
  • Assist the child/parent to reach the goal identified in the service plan by taking an active part in the procurement of services through referrals to the Special Child Health Services network and other appropriate agencies.
  • Be available to the family as a resource in crisis responding actively to complaints about services and providing objective information about alternatives for securing direct services.
  • Promote and facilitate communication among multiple providers serving the child/parent including the primary physician.
  • Monitor the services received by the child/family by reviewing with the parent the receipt of services and the childs progress toward the attainment of goals identified in the Individual Service Plan.
  • Provide initial screening and eligibility determination for the Catastrophic Illness in Children Relief Fund Program in accordance with the SCHS and the CIRCRF Program rules and regulations as per the SCHS letter of agreement.
  • Ensure compliance with SCHS criteria and procedures in the County Case Management Unit.
  • Provide required quarterly statistical reports to SCHS and the county.
  • Exhibit respect and support for and adherence to goals objectives policies and procedures approved by the Board of Education.
  • Program additional duties to those specified at the request of the Superintendent or his/her designee.
