Job Title: RN Inpatient Care Coordinator

Position Overview
The Inpatient Care Coordinator is responsible for supporting inpatient care coordination and utilization management activities within a critical access hospital setting. This role works collaboratively with an interdisciplinary healthcare team to ensure patient-centered care appropriate resource utilization regulatory compliance and safe timely transitions of care.
This position plays a key role in developing individualized treatment and discharge plans that align with patient needs payer requirements and available resources.
Key Responsibilities
Collaborate with a system-wide interdisciplinary team to support inpatient Care Coordination and Utilization Management
Coordinate the local Swing Bed / Transitional Care Program
Lead inpatient interdisciplinary team meetings
Review all inpatient admissions to develop individualized clinically appropriate care plans
Ensure treatment plans align with patient preferences available resources and regulatory and payer requirements
Collaborate with providers leadership and clinical support teams to ensure compliance with utilization criteria
Maintain accurate and timely documentation to support Utilization Management activities
Communicate effectively with payors regarding authorization and care coordination needs
Educate patients and families on healthcare benefits treatment plans and care options
Assess patients for psychosocial needs and initiate appropriate interventions and referrals
Develop and communicate discharge plans to ensure safe coordinated transitions of care
Support peer review activities as part of quality and compliance initiatives
Education Certifications & Licensure
Active unrestricted Registered Nurse (RN) license (Alaska or other U.S. state)
Bachelors degree in Nursing preferred
Case Management Certification from a recognized certifying organization (required within 24 months of hire)
BLS (Basic Life Support) required
ACLS (Advanced Cardiovascular Life Support) required
PALS preferred
High School Diploma or equivalent
Completion of required clinical and high-risk competencies within designated timeframes
Experience Requirements
Knowledge & Skills
Knowledge of medical record review medical necessity criteria insurance guidelines and regulatory compliance
Strong verbal and written communication skills
Excellent organizational and time-management abilities
Core Competencies
Ability to work collaboratively within an interdisciplinary healthcare team
Strong relationship-building skills with colleagues community partners and patients
Ability to manage multiple priorities while maintaining attention to detail
Physical Demands
Frequent standing walking sitting and use of hands and arms
Occasional climbing stooping kneeling crouching or crawling
Ability to lift and/or move up to 50 pounds
Required Certifications
Registered Nurse License (State-specific in good standing)
Basic Life Support (BLS) American Heart Association
Advanced Cardiovascular Life Support (ACLS) American Heart Association
Case Manager Certification (Commission for Case Manager Certification or equivalent)
