Job Summary

description:

Coordinates incoming office referrals and verifies insurance coverage. Coordinates incoming calls from patients family physicians and disciplines. Ensures that all personnel are informed of patients hospitalization and follow-up with patients admitted to other hospitals. Coordinates small program(s) with limited budget/impact. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work Shift 5 x 8s M - F 8am to 5pm. MUST have LVN certification. At least 1 year of experience. This will be a clinical intake coordinator for home health processing and scheduling referrals. No Call.
