Gross Weekly Pay: $2434.90
W2 Rate: $20/hour
Weekly Stipend: $1785

Position Overview

We are seeking a Licensed Master Social Worker (LMSW) to support inpatient medical services in a hospital setting. The LMSW will collaborate with interdisciplinary care teams to provide psychosocial assessments discharge planning and patient advocacy while ensuring continuity of care and compliance with regulatory standards.

Responsibilities

Conduct comprehensive psychosocial assessments for hospitalized patients
Provide discharge planning and coordination of post-acute services
Collaborate with physicians nurses case managers and interdisciplinary teams
Support patients and families with education resources and care planning
Address barriers to discharge and facilitate safe transitions of care
Document all assessments and interventions accurately in the electronic medical record
Ensure compliance with hospital policies ethical standards and regulatory requirements

Required Qualifications

Masters Degree in Social Work (MSW)
Active New York Licensed Master Social Worker (LMSW) license issued by NYSED
Minimum 1 year of LMSW experience
Hospital or inpatient experience required
