Job Summary (Physical Therapy Assistant):
- Provide physical therapy services to residents in collaboration with care providers to maximize functional abilities and independence.
- Perform therapeutic procedures under physician or licensed therapist supervision following established protocols and precautions.
- Implement treatment plans with specific long- and short-term functional goals adjusting as necessary in consultation with supervising therapist.
- Seek input from residents and caregivers to inform treatment sessions and ensure patient-centered care.
- Communicate changes in resident status or treatment issues promptly to the supervising therapist.
- Attend and participate in team meetings relevant to resident care.
- Monitor and document treatment effectiveness modifying care as needed for optimal outcomes.
- Maintain thorough timely documentation accessible to the care team and residents as appropriate.
- Recommend and instruct residents in the use of assistive devices to enhance mobility and independence.
- Educate residents and caregivers on therapy techniques and document teaching outcomes.
- Ensure compliance with Medicare Medicaid and other reimbursement policies.
- Contribute to the development and implementation of new rehabilitation programs and services.
- Maintain required licenses certifications (including BLS) and competencies; complete annual compliance training.
- Support department cleanliness and infection control procedures; assist with equipment and space maintenance.
- Demonstrate strong communication teamwork customer service analytical and time management skills.
- Uphold the mission and values of Covenant Health Ministry in all interactions.
- Perform other duties as assigned within the scope of the role.
Education & Experience Requirements:
- Associate degree in Physical Therapy Assistant required.
- Current PT Assistant license and Basic Life Support (BLS) certification required.
- Previous experience in long-term care or skilled nursing facility preferred.
Physical Demands:
- Must be able to lift/carry up to 20 pounds frequently; up to 50 pounds occasionally (team lift required above 50 pounds).
- Must be capable of sitting standing walking bending stooping twisting and reaching.
- Must be able to perform all essential functions with or without reasonable accommodation.
