Job Summary

Job Title: Pharmacist

Duration: 1-year base contract with the possibility of up to four (4) one-year option extensions

Shift Schedule: 8 9 hour shifts Monday Friday generally between 8:00 AM and 6:00 PM (varies by facility)

How many shifts needed per month: Approximately 173 hours per pharmacist per month
(Based on 2080 hours per year per pharmacist)

Credentialing / Onboarding time: 6 weeks to 8 weeks minimum

License:

Active unrestricted Pharmacist license valid in any U.S. state territory or District of Columbia

Certifications:

Current Basic Life Support (BLS) certification

Experience needed:

Minimum 2 years recent experience providing similar pharmacist services
Documented past performance placing pharmacists in comparable roles within the last 2 years

Special Skills needed:

Clinical pharmacy services (outpatient and inpatient)
Pharmacokinetic dosing and management of narrow therapeutic index drugs
IV admixture and sterile compounding
Drug interaction screening and medication therapy management
Patient counseling across diverse and multicultural populations
Ability to work independently in a non-personal services contract environment
Proficiency with EHR/RPMS systems

Responsibilities:

Interpret prescriptions; compute dosages; compound and dispense medications
Review patient records for therapeutic appropriateness and drug interactions
Counsel patients on medication use precautions and storage
Consult with providers on dosing contraindications and alternatives
Manage pharmacy inventory storage conditions and expiration control
Participate in clinical consultations audits and quality improvement activities
Maintain compliance with IHS policies HIPAA and federal regulations
Rotate between SFIHC and Santa Clara Health Center as required
