DescriptionThe Trauma Assessment Clinician will conduct behavioral assessments on children in care to determine any indicators of complex trauma and recommend appropriate services that address diverse needs. This position will specialize in children who are a part of the foster boarding home special medical foster care and Health Homes programs.
Primary Responsibilities and Essential Functions of Position:
- Conduct initial behavioral assessments on children and youth who are directly placed into foster homes after being remanded
- Contact case planning teams placement resources and other stakeholders to arrange any appointments or consultations.
- Meet with the Medical Director to discuss any diagnoses or conditions that may impede the functioning of clients.
- Complete and upload all details of assessments to the Electronic Health Record (EHR).
- Conduct functional impairment assessments on children who have been exposed to complex trauma:
- Undergo training to become approved to use the CANS-Trauma Comprehensive tool.
- Perform case record reviews in order to obtain applicable information that is needed to inform assessments.
- Collaborate with other agency staff or specialists in order to utilize historical and current available information.
- Use multiple New York State (NYS) forms and clinical judgment to make recommendations on appropriate and intensive
- services.
- Provide in-house counseling services to children who are determined to have complex trauma until external services are located
- Establish treatment goals for children and youth based on the applicable categories of traumatic exposure and impairments.
- Refer children to supportive services to address the impact of traumatic experiences.
- Complete all required documentation including but not limited to session notes treatment plans psychosocial summaries billing documentation and client correspondence using the Electronic Health Record (EHR).
Qualifications- Masters degree (LCSW/LMSW) required
- Minimum of two (2) years work experience providing clinical services in a mental health or residential setting for youth and children required
- Experience working with youth and children exposed to trauma preferred
- Ability to speak multiple languages preferred
- Fluency computer software applications required including Microsoft Office and web based applications
