Your role and responsibilities

We are looking for an experienced enthusiastic practitioner with a positive mindset to join the West Flexible care & Support Team in the West of Suffolk.

  • Your role will see you work alongside colleagues both within the organisational and outside including health colleagues allocations lead Business Finance and Service Development and Contracts teams and wider partners putting you in an ideal position to develop build and help your experience in Adult Social Care.
  • The main purpose of this role is to work with adults to identify and deliver good quality outcomes to support and maximise independence and wellbeing.
  • You will be working as part of a team to promote personalisation independence wellbeing and choice to adults and to carry out asset-based assessments/reviews. Predominantly this position will be a generic role within the Team but also to cover our Discharge to Assess and Community Assessment Beds Safeguarding Reviews and Continuing Healthcare.

You will need

  • You will need experience of creating reablement plans in line with signs of safety solution focused approach.
  • This organisation positively encourages the use of technology to communicate and engage but in this role you will need to operate across a wide and rural area so you must either hold a full and current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.

The team

You will work within one or more area teams in a particular geographical area of Suffolk as we progress to the emerging new operating model and locality-based alliances. The area that you work in will have a number of area teams and will align to the clinical commissioning group footprint. You will work within the area team aligned to Integrated Neighbourhood Teams and deliver social work and trusted assessor services some of which may be complex situations requiring experience expertise and skill to maximise independence and wellbeing.

You will work with local Contract and Service Development Teams and managers and share information and good practice to inform future commissioning models and new ways of working.
